3) Goals and expectations:
What does success mean to you regarding this situation? What are the measurements that will indicate to you that we've fulfilled your expectations? Where do you want to see your adolescent in the short term and into the long-range future?
4) History:
What are the behavior patterns, themes, trends, trauma (when applicable), that may have contributed to the current challenging situation that is facing your adolescent and family? What are your adolescent's hobbies, strengths, talents, and weaknesses that we need to know about?
5) Options:
We provide some perspective regarding the range of possibilities given the information you provide to us. We offer you an overview of the entire "industry" so we can be a better team working together.
